- The corporation was dissolved for not paying the incorporation fee.

Please note a few things before completing the form:
- You must include a cover letter with your request to revive a not-for-profit corporation. See Instructions for Form 4015 – Articles of Revival for more details on what to include in the cover letter.
- Corporations Canada can refuse to issue a Certificate of Revival if there is a valid reason to do so. Fees are not reimbursed if the request is not approved or is withdrawn by the applicant. There are a number of circumstances where Corporations Canada will refuse to issue a Certificate of Revival, including (but not limited to):
